To enroll for the 2023-2024 school year, please contact the Enrollment Center at 740.223.4422 or 740.223.3770
The following is a list of documents required for ALL enrollment:
- Birth Certificate
- Parent/Guardian Photo ID
- Proof of Residence **
- Immunization Records
- IEP, if applicable
- Custody agreements, if applicable
- Grandparent POA (no other POA's are acceptable), if applicable.
**Proof of residence must be one of the following:
Current gas, electric or water bill that has you or your spouse's name and address on it.
Current Mortgage statement, lease/rental agreement (with parent/guardian name on document)
Current Bank statement
Current payroll check with mailing address and parent/guardian name on it
**If you are living at someone else's residence, and do not have any type of mail with your current address, you and the current owner of that residence will need to complete an address affidavit form. You can download this form from the sidebar under Downloadable Forms for Enrollment.
All Preschool enrollments will also need to provide INCOME. Further details will be available when you call the Enrollment Center to continue the enrollment process.
