You will need the following documentation to enroll your student.
- Birth Certificate
- Your photo ID
- Proof of Residence - Rental/Lease Agreement, Mortgage Statement, Purchase Contract to buy property, Current Public Utility Bill, Current Government Mailing, Current bank Statement, or Current Payroll Check
- Shot Records
- Custody/Legal Agreements, This includes DIVORCE Decrees, if applicable
- Copy of students IEP/ETR/504 if applicable
